Parents of Rising Seventh Graders:
Effective July 1, 2019, all students are now required to have a Tetanus, Diphtheria Pertussis (Tdap) booster vaccine PRIOR to entry into the SEVENTH GRADE.
Documentation of this booster vaccine must be provided to Read Mountain Middle School for enrollment into the seventh grade.
You may drop off shot records between the hours of 8 a.m. and 4 p.m. Monday through Friday. You may also fax your shot record to Read Mountain Middle School to the attention of Donna Price at 540-966-8656.
The Botetourt County Health Department will offer drive-thru clinics on the following dates:
August 17, 2020 Read Mountain Middle School 4pm -7 pm
August 20, 2020 Botetourt County Health Department 1 pm- 6 pm
You must pre-register for these dates by calling the Health Department at 540-473-8240. At pre-registration you will be given an appointment for your child. The clerical staff will obtain your insurance information and inquire about your child’s immunization record.
Directions for entering the drive thru line:
At the Botetourt County Health Department, please enter the front parking lot and drive around to the rear entrance across from the library.
At Read Mountain Middle School, please enter the upper parking lot and continue around to the car rider line.
